Ontario IT Funding York-Med's MD Suite is a Certified Clinical Management System (CMS / EMR) The IT Physician Program – How it Works? What is the IT Physician Program? The purpose of this IT Funding is to assist Primary Care Physicians with the transition to a computerized practice. The implementation of these technologies will help to: · integrate electronic medical records, scheduling and billing · keep up-to-date, accurate and secure patient records · share integrated, legible patient information with colleagues · make better decisions by having access to the latest drug alerts and research · practice pro-active preventive care · automate administration tasks Do I Qualify? Eligible Groups Include: · Family Health Network (FHN) · Family Health Team (FHT) · Health Service Organization (HSO) · Primary Care Network (PCN) · Family Health Organization (FHO) · Group Health Centre (GHC) · Rural and Northern Phsyician Group Agreement (RNPGA) If you are a member of a Family Heath Group (FHG) you may be eligible to receive funding, please check with andrea.flint@york-med.com. How Much Is Available to Me? The Comprehensive IT Funding Package Includes: + An Initial Grant of $4500.00 Per Participating Physician
+ A Bonus of $2500.00 Per Participating Physician Once 2/3rds or 600 EMR patients + A Monthly Payment of $600.00 per Participating Physician for 36 Months = For a grand total of $28,600.00 per Participating Physician*
How Can York-Med Help to Secure These Funds?** 10 Steps to $28,600.00… 1) Meet with a York-Med HealthCare Solutions Consultant for a Free Needs Analysis 2) York-Med will Help Determine Which Software Solution will Best Address Your Practice Needs 3) Your York-Med Consultant will Assist with the Submittal of a ‘Letter of Intent’ 4) York-Med will Help Complete the Required SSHA Documents for Your Complimentary Network 5) A ‘Scope of Work’ Document will be Completed & Submitted to Your Transition Specialist 6) The Network Order Form & rVPN Form are Submitted to the SSHA 7) The ‘Vendor Contract Declaration’ Form and the ‘Electronic Funds Transfer’ Form are Processed 8) York-Med will Install, Test, & Train you on Your New System 9) A ‘Go Live Form’ will be Completed and Submitted on Your Behalf 10) Once your EMR is Up and Running the ‘Performance Recognition Declaration’ Form will be completed for the $2500 Per Physician Bonus
